Adrenarche, gonadarche, thelarche, spermarche, menarche, pubarche and puberty: do you understand these terms? This paper is useful for the general reader. Houghton L C ( JAMA Pediatr 2026;180(8):813-814. Doi:10.1001/jamapediatrics.2026.1461) has published a viewpoint paper and Lucina found it a very valuable summary of the terms and, often associated confusion, of those terms used in endocrinology to describe features of the puberty process. Many generalists will diagnose and manage thelarche and adrenarche without referral to endocrinology. Precocious puberty, especially in boys, does need a referral to endocrinology for further imaging and investigation. What about the other terms; do you understand the physiology and pathophysiology? This is a great read and helps clarify these issues (to some extent!). There is a table defining adrenarche, gonadarche, pubarche and puberty.
