Blood biomarkers are transforming clinical practice for Alzheimer's disease.1 Plasma phosphorylated tau-217 (p-tau217), a highly sensitive and specific marker of Alzheimer's disease pathology,2 has entered the diagnostic pathway in many specialist centres, facilitating early diagnosis and access to treatment. In research settings, ultrasensitive multiplex assays can now assess biomarker trajectories in great detail during disease progression and in response to therapy. In The Lancet Neurology, Rui Mu and colleagues present findings from a longitudinal study of 197 individuals with early symptomatic Alzheimer's disease receiving lecanemab every 2 weeks as part of their clinical care.
