Interim results from the phase 3 HARMONi-GI1 trial demonstrate the first-in-class PD-1×VEGF bispecific antibody ivonescimab, combined with gemcitabine and cisplatin, significantly improved overall survival compared with durvalumab plus the same chemotherapy backbone in advanced biliary tract cancer. The regimen may represent a new standard of care for first-line advanced biliary tract cancer, though questions regarding patient selection and applicability beyond Chinese populations remain.
