Yellow fever is an important public health problem, but vaccine manufacturing cannot be scaled up easily, making it necessary to use existing vaccine stocks to protect as many people as possible. Fractional doses, usually one-fifth of a vial, have been used during outbreaks. The use of fractional doses is supported by evidence in adults showing immunogenicity1,2 and is endorsed by WHO.3 Yellow fever vaccine is routinely given to infants and young children in endemic areas, but whether fractional dosing is effective in children was unclear until recent trials from 2023 and 2026 addressed this question.
