FIGO 2021 aligned vulvar nodal-metastasis size definitions with the cervical system and subdivided stage III into A/B/C.
When to use
Staging and management direction for vulvar carcinoma under the FIGO 2021 system.
How it works
I confined to vulva (IA ≤ 2 cm and ≤ 1 mm invasion / IB > 2 cm or > 1 mm, nodes negative); II lower 1/3 urethra/vagina/anus; III upper 2/3 structures or nodes (IIIA ≤ 5 mm or upper 2/3, IIIB > 5 mm, IIIC extracapsular); IVA fixation/fixed-ulcerated nodes, IVB distant.
Key points
- FIGO 2021 aligned vulvar nodal-metastasis size definitions with the cervical system and subdivided stage III into A/B/C.
- IA (invasion ≤ 1 mm) carries a very low nodal-metastasis risk and generally needs no groin node assessment.
- Sentinel-node biopsy is standard for eligible IB+ unifocal tumours with clinically negative groins.
- Positive margins or nodes drive adjuvant (chemo)radiation to the groins/pelvis.

Worked calculation
The values below come from this tool's own example placeholders and are computed server-side with the formula shown on this page, so the arithmetic can be checked quickly. It demonstrates how to substitute values only — it is not clinical advice and not a real case.
| Primary lesion (when confined to vulva/perineum) | IA — ≤ 2 cm and stromal invasion ≤ 1 mm |
|---|---|
| Local spread | Lower 1/3 urethra/vagina/anus (→ II) |
| Inguinofemoral nodes | Metastasis > 5 mm (→ IIIB) |
| Fixation / distant | Fixed to pelvic bone (→ IVA) |
→FIGO stageStage IVA
- FIGO 2021 stage:Stage IVA (fixed to pelvic bone, or fixed/ulcerated nodes)
- Management direction:Inguinal node-positive or locally advanced: surgery + inguinal-pelvic radiation ± concurrent chemotherapy (cisplatin); definitive chemoradiation if unresectable; IVB systemic therapy ± palliation
- Notes:The 2021 nodal metastasis definitions align with cervical cancer; stage III is subdivided A/B/C (≤ 5 mm / > 5 mm / extracapsular); IVA includes fixation to pelvic bone or fixed/ulcerated nodes; invasion-depth definitions have been updated
| Primary lesion (when confined to vulva/perineum) | IA — ≤ 2 cm and stromal invasion ≤ 1 mm |
|---|---|
| Local spread | None |
| Inguinofemoral nodes | None |
| Fixation / distant | None |
→FIGO stageStage IA
- FIGO 2021 stage:Stage IA (≤ 2 cm and invasion ≤ 1 mm (nodes negative))
- Management direction:IA (invasion ≤ 1 mm): wide local excision, generally no inguinal node assessment needed (very low nodal metastasis risk)
