LimitsDoes not replace a full transplant evaluation. · Lab error is amplified by logarithms. · Acute liver failure is not scored by PELD alone.

Clinical takeaway

For age <12 years.

When to use

Stratify pediatric liver disease and support transplant teaching. Use MELD from age 12.

How it works

PELD = 10 × [0.480 ln(bilirubin mg/dL) + 1.857 ln(INR) − 0.687 ln(albumin g/dL) + 0.436×(age<1 y) + 0.667×growth failure] Bilirubin/INR floored at 1.0; albumin capped at 3.5 g/dL.

Key points

  • For age <12 years.
  • Albumin is a negative term, capped at 3.5 g/dL.
  • Infant age and growth failure add points.
