Race-free 2021 equation, reported per 1.73 m².
When to use
Assess and stage chronic kidney disease (with albuminuria).
How it works
CKD-EPI 2021: 142 × min(SCr/κ,1)^α × max(SCr/κ,1)^−1.200 × 0.9938^age × (1.012 if female); κ=0.7/0.9, α=−0.241/−0.302.
Key points
- Race-free 2021 equation, reported per 1.73 m².
- Confirm eGFR < 60 over ≥ 3 months for CKD.
- Combine with albuminuria for KDIGO category.

Eur Heart J. Published online August 28, 2026. doi:10.1093/eurheartj/ehag098). It recommends staging with a validated creatinine-based eGFR equation, confirming chronicity with two eGFR and uACR measurements at least 3 months apart, and considering cystatin C–based eGFR when needed. For European populations it prefers the 2009 CKD-EPI equation without the race coefficient, so it is not the basis for the equation on this page. This page keeps CKD-EPI 2021 within the KDIGO 2024 framework; the equation and stage cut-offs are unchanged.

Frequently asked questions
- Does eGFR Calculator (CKD-EPI 2021) upload patient data? Where is it calculated?
- Calculation runs locally in the browser; inputs and results are not uploaded. For licensed clinicians only; interpret with history, investigations and local protocols — not a diagnosis or prescription.
- How is eGFR Calculator (CKD-EPI 2021) calculated in practice? Can you show a worked example?
- Inputs: Serum creatinine 1.0, Creatinine unit mg/dL, Age 60 years, Sex Male → Result: eGFR 86 mL/min/1.73m²(Creatinine used: 1 mg/dL, CKD GFR category: G2 (60–89), Equation: CKD-EPI 2021 (creatinine, race-free)) Inputs: Serum creatinine 80, Creatinine unit µmol/L, Age 60 years, Sex Female → Result: eGFR 73 mL/min/1.73m²(Creatinine used: 80 µmol/L, CKD GFR category: G2 (60–89), Equation: CKD-EPI 2021 (creatinine, race-free))
