- Who it is for
- Adults in whom IV unfractionated heparin has already been chosen, e.g. high-risk PE, possible reperfusion, severe renal impairment, or severe obesity.
- Who it is not for
- Most patients without high-risk PE: LMWH, fondaparinux, or a DOAC is preferred for initial anticoagulation. Known or suspected HIT: no heparin. Children: do not apply this adult regimen.
- Formula and variables
- Weight (kg) and indication. Bolus and starting infusion follow this page's formula and are not repeated here. The VTE row follows the Raschke 1993 weight-based nomogram; the ACS row follows the 2014 AHA/ACC NSTE-ACS guideline.

Methodology · worked example, guidelines & full notes
Recheck aPTT 6 h after starting and after each rate change.
When to use
Choose an initial weight-based UFH regimen; subsequent rates are titrated by aPTT or anti-Xa and your institutional nomogram.
How it works
VTE: 80 U/kg bolus + 18 U/kg/h. ACS anticoagulation depends on the reperfusion strategy; see the cited guideline (2025 ACC/AHA ACS guideline, PMID 40014670). Target aPTT control or anti-Xa.
Key points
- Recheck aPTT 6 h after starting and after each rate change.
- Monitor platelets for heparin-induced thrombocytopenia (HIT).
- Watch for bleeding and impaired renal function.
- Always titrate by your institution's nomogram.

FAQ
Frequently asked questions
- What are the limits and cautions when using Unfractionated Heparin (UFH) Weight-Based Dosing?
- For licensed clinicians and clinical researchers. Interpret results with history, investigations and local protocols; not a diagnosis or prescription, and not a substitute for multidisciplinary decision-making or local guidelines.
- Does Unfractionated Heparin (UFH) Weight-Based Dosing upload patient data? Where is it calculated?
- Calculation runs locally in the browser; inputs and results are not uploaded. For licensed clinicians only; interpret with history, investigations and local protocols — not a diagnosis or prescription.
