Methodology · worked example, guidelines & full notes
Only categories 0/1/2 are used for screening; 3/4/5/6 follow a diagnostic work-up.
When to use
Translate an assigned BI-RADS category into the correct management pathway.
How it works
0 incomplete → additional imaging; 1/2 → routine screening; 3 (< 2%) → short-interval follow-up; 4A/4B/4C (2–95%) and 5 (≥ 95%) → biopsy; 6 → definitive treatment.
Key points
- Only categories 0/1/2 are used for screening; 3/4/5/6 follow a diagnostic work-up.
- Category 3 (probably benign) means malignancy < 2% and short-interval follow-up.
- A benign biopsy discordant with imaging in category 4C/5 needs further action.
- Category 4 is sub-divided 4A/4B/4C by the physician's malignancy estimate.

Worked calculation
The values below come from this tool's own example placeholders and are computed server-side with the formula shown on this page, so the arithmetic can be checked quickly. It demonstrates how to substitute values only — it is not clinical advice and not a real case.
| BI-RADS category | 4A — Low suspicion |
|---|
→ManagementBiopsy
- Category:BI-RADS 4A: Low suspicion (malignancy 2–10%)
- Management:Image-guided needle biopsy; a benign result concordant with imaging is acceptable with no further action
- Note:BI-RADS management falls into four groups: additional imaging, routine screening, short-interval follow-up, biopsy; only 0/1/2 are used for screening, 3/4/5/6 after a diagnostic work-up
| BI-RADS category | 0 — Incomplete |
|---|
→ManagementAdditional imaging / prior comparison
- Category:BI-RADS 0: Incomplete assessment
- Management:Additional imaging (spot/magnification views, ultrasound) or comparison with priors, then assign a final category
- Note:BI-RADS management falls into four groups: additional imaging, routine screening, short-interval follow-up, biopsy; only 0/1/2 are used for screening, 3/4/5/6 after a diagnostic work-up

FAQ
Frequently asked questions
- What are the limits and cautions when using Breast Imaging BI-RADS Category & Management?
- For licensed clinicians and clinical researchers. Interpret results with history, investigations and local protocols; not a diagnosis or prescription, and not a substitute for multidisciplinary decision-making or local guidelines.
- Does Breast Imaging BI-RADS Category & Management upload patient data? Where is it calculated?
- Calculation runs locally in the browser; inputs and results are not uploaded. For licensed clinicians only; interpret with history, investigations and local protocols — not a diagnosis or prescription.
- How is Breast Imaging BI-RADS Category & Management calculated in practice? Can you show a worked example?
